Marine Incident Investigation That Finds the Root Cause and Prevents Recurrence
Most onboard incident reports stop at "crew failed to follow procedure". Our Master Mariner-led investigators go one question further: why the failure made sense at the time, and what your fleet has to change so it does not happen on the next ship. You get a VDR-backed timeline, a structured root cause analysis and corrective actions your management system can close.

The question is no longer what happened, but what changed afterwards
Inspectors, oil majors and charterers now look past the incident itself. They want to see that the investigation found real causes and that the fleet learned from it.
Accidents and hazardous occurrences must be reported, investigated and analysed, with corrective action taken to prevent recurrence.
Operators are assessed on the quality of their investigations, the depth of root cause analysis and whether lessons are shared across the fleet.
Inspectors increasingly test whether past incidents produced real change in procedures, training and behaviour on board.
Dry bulk operators face the same expectation of structured incident investigation, analysis and follow-up.
Incident history feeds the vessel's safety profile, so a repeat event costs more than the incident itself.
What a shallow investigation costs you
The most expensive pattern in ship operation is the repeat incident: same root cause, different vessel, larger consequence.
- "Human error" closes nothing
A report that ends at crew error leaves the conditions that produced it in place: fatigue, unworkable procedures, gaps in training or supervision.
- Evidence lost in the first hours
VDR data overwrites, memories fade and the scene changes. Without fast preservation the real sequence of events can never be proven.
- Corrective actions that miss
Actions aimed at symptoms add another circular to the SMS but leave the barrier that failed exactly as it was.
- Lessons that stay on one ship
If the learning is not turned into fleet guidance and training, sister vessels carry the same risk unchanged.

Everything you get with an incident investigation
A report your management system can act on, not a PDF that gets filed.
VDR-backed timeline
A time-stamped reconstruction of events from bridge audio, radar, ECDIS, helm and engine orders, alarms and VHF wherever VDR data exists, cross-checked against logs and statements.
Structured root cause analysis
Recognised methods such as HFACS for human factors, BowTie for barrier failures and fault tree analysis where causes branch, separating immediate causes from underlying ones.
Human factors review
Technical, procedural, organisational and behavioural contributors identified, including fatigue, workload, supervision and procedure design.
Corrective and preventive actions
Clear actions linked to each root and contributory cause, written so your team can assign owners and track them to close-out.
Fleet lessons-learned brief
A short, shareable summary for sister vessels and the office, so one incident improves the whole fleet.
Strict confidentiality
Investigations are handled in strict confidence. Information is shared only with authorised client personnel, data is handled under ISO 27001:2022 controls, NDAs are available, and findings are never disclosed to third parties without your consent.
Training-ready extracts
Audio and video clips and case material from the investigation, ready to use in your own crew training.
From first call to closed-out actions
We guide the vessel from the moment you contact us, so evidence is secured before it is lost.
- 1
Immediate engagement
Share the incident details and we send a preservation checklist straight away: save the VDR record first, then logs, alarm and ECDIS records, photographs and short statements.
- 2
Evidence collection
VDR data, logbooks, alarm records, ECDIS tracks, checklists and witness accounts are collected and reviewed. Where the incident warrants it, an investigator attends the vessel.
- 3
Structured analysis
We build the factual timeline, then apply root cause methods to find why the barriers failed and what conditions allowed it.
- 4
Report and debrief
You receive the report with root causes and corrective actions, and we debrief your office and crew so the lessons are understood, not just read.
A preliminary view of what happened and the immediate actions needed comes first. The time to the full report depends on the complexity of the incident, the quality of the data and crew availability for interviews, and we agree it with you at the start.
Why fleet operators choose Synergetic to investigate
We investigate to prevent the next incident, not to assign blame for the last one.
Mariners investigating mariners
Investigations are led by Master Mariners with command experience, who understand why decisions made sense on the bridge or on deck at the time.
VDR expertise at scale
2,200+ remote VDR analyses mean our team reads bridge recordings every day and reconstructs events from objective data, not opinion.
Confidential by default
Your incident stays yours. Only authorised client personnel see the evidence and findings, our data handling is ISO 27001:2022 controlled, and we sign NDAs on request.
Systemic, not personal
Individual actions are examined to understand the conditions behind them, because those are the factors an operator can actually fix.
Learning built in
Every investigation ends with fleet guidance and training material, closing the loop the ISM Code and TMSA 3 expect.
Investigations backed by experience
Evidence first
Findings are built on VDR data, records and statements, so conclusions can stand scrutiny from management, vetting inspectors and auditors.
Corrective actions that close
Each action is tied to a cause, so your team knows exactly what to fix and how to show it has been fixed.
Lessons the fleet remembers
Officers learn from recordings of a real event, which changes behaviour in a way a circular does not.

Questions about incident investigation
Tell us about your fleet and we will answer directly.
Request an InvestigationWhat is a marine incident investigation and root cause analysis?
A marine incident investigation establishes what happened, why it happened and what must change to prevent it happening again. Synergetic Shipping Technologies reconstructs a time-stamped timeline from VDR data, records and statements, applies structured methods such as HFACS, BowTie and fault tree analysis, and delivers root causes with corrective and preventive actions your fleet can close.
When should we bring in an external investigator instead of investigating in-house?
When the potential consequence was high regardless of the actual outcome, when the same type of incident keeps recurring, when internal findings keep landing on human error without explaining it, or when the investigation will be closely reviewed by an oil major, RightShip or your P&I Club. Independence also matters where internal reporting lines could be part of the causal picture.
What evidence should we preserve immediately after an incident?
Save the VDR or S-VDR record first, because it overwrites on a fixed cycle. Then secure deck and engine logbooks, ECDIS and alarm records, relevant checklists and permits, photographs of the scene before anything is moved, and short individual statements while memories are fresh. We send a preservation checklist on first contact.
Will the investigation satisfy ISM Code and TMSA 3 requirements?
The report is structured around the ISM Code cycle of investigation, analysis, corrective and preventive action and follow-up. For tanker operators it is written to stand up to TMSA 3 expectations on investigation quality, root cause depth and fleet-wide learning, and the same standard serves DryBMS and vetting reviews.
Does the investigation blame individual crew members?
No. Individual actions are examined to understand why they made sense at the time, looking at fatigue, procedure design, training, supervision and commercial pressure, because those are the factors an operator can fix. A report that ends at crew error changes nothing.
Is the investigation kept confidential?
Yes. Synergetic Shipping Technologies handles every investigation in strict confidence. Information is shared only with authorised client personnel, evidence and reports are handled under our ISO 27001:2022 information security controls, NDAs are available on request, and findings are not disclosed to any third party without the client's consent.
